This includes; personal details, medical history, payment information and any form of communication between client and practitioner.

We obtain all our information directly from you or your health practitioners of whom you have seen and authorised to disclose to us. The information we collect from you is used only for the purposes of clinical care. We will disclose your personal health information to practitioners with your consent. All information we collect from you will be protected with appropriate safeguards and security. Any personal health information that in no longer required for clinical care will be destroyed.

Accessing Your Information:
We will give you access to the information we have collected and retain about you within a reasonable time, upon presentation of a written request and satisfactory personal identification. There will be no charges associated to this service.
